unless a locating pin fell out ?
I'd say it was something other than the piston itself

bored/honed for that exact piston ?
by a competent machinist ?
what piston to cylinder clearence ?
ports adequately chamfered ?
rings facing correctly ?
leakdown tested and passed ?
3+ heat cycles and retorqued base/head nuts
warmed up fully before riding ?
jetting correct ?
crank good ?

just suggestions so it doesn't happen again
 
Jetting was not right. Ordered a newer carb with the specs you sent me but seeing my daughter riding her little quad around the yard made me want to do a small quick ride before installing new carb and yea.. ending result it started for about 2 mins and i cracked on it and pow! Never started again. Just pulled motor tonight to see how bad damage is. Ill keep update on anything else i find.

Oh, and not sure if it was a suitable mech that did original build. Bought a spare motor off someone out in oklahoma. 240 kit running for $250. Couldnt pass on cheap spare motor. This ended up being the motor i used
 
This could be the result of breaking in an engine at more than 3/4 throttle while there is still fresh crosshatching in the bore.

The ring lands become fatuiged from having to take the extra pressure applied to them whilst being forced out toward an abrasive surface!

The ring lands become weakened especially at the ring ends, and then some hours down the track break and release their hold on the ring.

I have seen this happen numerous times, hence the reason I promote a soft break in period!
